This is arguably the rarest item in the game which is exacerbated by the difficulty of farming for it. The only other time I've heard of this sword dropping was for a horde guild some time during early pre-Burning Crusade, in which they received the drop from Molten Core or Onyxia (which KingDarius confirms). If you were on an epic quest to obtain this sword, your absolute best bet would be a server whose Ahn Qiraq gates are not yet opened (if such a server is ever opened these days) and prepare a raid to farm these mobs until the event is over. The drop rate for a Teebu's from the Lieutenant could very well have been 20% or close to it. I did not witness a Teebu's drop myself, but did see an Alcor's Sunrazor, Cloudkeeper Legplates, a Lady Maye's Pendant, and two Hand of Edward the Odds drop, to name what I can recall.
The bosses did indeed drop epics all of the time, and the epics were indeed world BoEs. All of the bosses there required a large number of people to kill and were only available for a few hours. So maybe the item was finally put into loot tables around when AQ was put into the game. If I remember correctly, the person that placed it on the auction house was the guild bank alt of one of the leading PvE guilds, and I believe they said they got it while farming the elite Dragonkin in Seradane in Hinterlands for The Nightmare's Corruption quest ( ). I didn't find it myself, I bought it off the auction house for 1500g and ended up reselling it for a little over 3000 (after many tries for 5000+, but this was when a thousand gold actually meant something). I had one on my mage ~2 years ago, around when the AQ quest line was introduced.
